ExitCare provides peer-reviewed patient education and discharge instructions to support clinicians, empower patients and help providers meet their patients™ educational needs.
Focused on improving patient care and outcomes, Elsevier and ExitCare solutions give healthcare providers the tools and information necessary to ensure that patients are discharged in accordance with The Joint Commission and Meaningful Use requirements. ExitCare™s products, incorporated into Elsevier™s portfolio of clinical decision support (CDS) content and tools, will help healthcare providers improve the delivery of healthcare information and services by interdisciplinary care teams across all care environments.
